Licensed Clinical Social Worker – Behavioral Health
Hanover, Maryland

General Summary of Duties: This position is responsible for providing clinical social work services in an integrated behavioral health program. The position provides social services to apply the theories, knowledge, procedures, methods, and ethics derived from a formal educational program in social work to restore or enhance social functioning of individuals and family groups through assessment, planning, intervention, case management, evaluation, and advocacy to maximize well-being. Service delivery will be primarily telephonic initially, and/or meeting patients in a clinical office setting.

Supervision Received: Lead, Clinical Coordinator

Supervision Exercised: None

Typical Physical Demands: Requires sitting, some standing, stooping, and stretching. Occasionally may lift up to 30 pounds. Requires sufficient hand-eye coordination and manual dexterity to operate a keyboard, photocopier, telephone, calculator, and other office equipment. Requires normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.

Typical Working Conditions: Work is typically performed in an office environment and could involve contact with staff and/or patients.

Primary Duties/Responsibilities Include but not Limited to:

  • Participate in care coordination as member of a team of health professionals supporting the needs of patients in primary care.
  • Employ a social work intake process that quickly identifies major social and behavioral threats to health and communicates that information to case management team and primary care practitioners.
  • Assess impact of social determinants of health on patient well-being and effectively refer patients to community resources, as needed and/or as requested by the patient.
  • Engage and motivate patients to develop treatment options/goals that are congruent with identified psychosocial needs in collaboration with patient’s primary care provider.
  • Communicate information regarding contact with patient to the health care team through documentation system and team meetings.
  • When clinically appropriate, refer patients to more intensive behavioral health services.
  • Communicate with psychiatric consultant and substance use providers and update team through the medical record.
  • Complete all documentation and administrative requirements including timely entry into the medical record.
  • Assist in development of policies, protocols, or procedures as requested.
  • Performs other duties as assigned.

Minimum Qualifications:

Education: Master’s Degree in Social Work (MSW)

Licensure: Licensed Social Worker – Clinical (LCSW-C) in the State of Maryland. Will consider a Licensed Master Social Worker (LMSW) in the process of finalizing their LCSW-C.

Experience: Two (2) years clinical social work experience under supervision of LSCW-C.

Preferred Experience: Experience in a health or behavioral setting is preferred. Experience training, coordinating work, and monitoring the work of others preferred.

Maryland Primary Care Physicians is a busy, growing, established family practice group. We are currently seeking a full-time Licensed Clinical Population Health Social Worker. This position would be responsible for providing clinical social work services in a population health environment. Service delivery will be primarily telephonic initially, and/or meeting patients in a clinical office setting. This position requires a broad-based knowledge of mental health conditions, psychosocial issues and chronic diseases which impact health. Utilizes evidenced based practice and national standards to optimize the mental health of the high-risk medical and psychiatric population and to maintain these patients in the community, avoiding hospitalization when appropriate through timely behavioral help follow up and intervention.

Essential Functions:
